The community at Ysguborwen can offer consistency in care due to the fact we have separate areas, each offering specialist long term care. These areas consist of:
LILAC: provides support for people who need more help in personal care and have a diagnosis of dementia. Within Lilac we embrace the lived experience of people with a dementia and take on the language of person centred care. We focus on seeing the person as a whole and on connecting to and meeting people’s emotional needs alongside their family. We focus on ‘being’ rather than ‘doing’, as advocated by David Sheard.
HEALTHCARE: this unit provides continuing healthcare, nursing and personal care and is supervised at all times by qualified nurses. Encouragement is provided to ensure residents maintain their independence.
Our philosophy is to ensure that the people we care for feel this is their home; so our main aim is to ascertain what they individually require, and then strive to meet thier needs
A person centred approach here at Ysguborwen is about our community of people-those we are supporting and our staff coming together in a common purpose-the purpose of making life worth living and feeling valued by each other.
To enable us to offer this, we work a ‘key worker and named nurse system’, which means each individual resident will have a named member of staff who will keep close contact with you to find out what you want and need and relate this to the rest of the care staff so they are aware of your needs.
In line with maintaining our ‘Investors in People’ (which is a National Audit for a continual learning environment), training our staff has a high priority. As well as working towards QCF Levels 2, 3 and 4 (which are nationally recognised standards of achievement in the caring profession), varied and diverse training is frequently delivered.
It is a pre-requisite of any employee that they have the enthusiasm and ability to work towards these achievements. This plays a large part in ensuring our standards of care are the highest.
All usual national health services are available; your choice of local doctor will be adhered to; if moving area, your own doctor may not be able to continue care due to distance, in which case the Heath Service will allocate a local doctor. Other visiting services include Chiropody, Physiotherapy, Consultants, Dentists, and Opticians.
Ysguborwen Community has all the usual facilities you would expect; lifts; nurse call system; fire alarm system; TV rooms and telephone facilities. For the comfort and safety of all residents we do have a primarily non smoking policy, but does enable existing suitable assessed residents access to a smoking room.
Visitors are actively encouraged to visit whenever they wish and to participate in all in-house and outside activities. Again it is the resident’s choice that dictates the needs, not the routine of the home.
